Just having a look at story discussions and it seems that you have to look at some website and comics to get the full picture. That's bad story telling and a load of old bollocks.
It bugs me when you have to go off and do your own research because a game is lacking. It's the same with secrets in some games - sometimes you might fluke an unlock of something or be given hints, but then with other stuff, you have no chance. For example, here's how you unlock 'The Lost' character in Binding of Isaac -
1. Isaac must die to a Mulliboom in The Basement or The Cellar.
2. Magdalene must die to her own bomb in The Caves or The Catacombs.
3. Judas must die to any Mom attack in The Depths or Necropolis. Being killed by any monster spawned by Mom will spoil the entire attempt at unlocking The Lost.
4. Azazel must die to any Satan attack in Sheol. Being killed by The Fallen in the boss room or a Kamikaze Leech will spoil the entire attempt at unlocking The Lost. Being killed by using The Bible doesn't count as a Satan kill.
All in consecutive runs. Apparently there was a way of revealing puzzle pieces during a run and the initial idea was that the community would share these pieces and complete the puzzle to figure out the above, but as far as I'm aware, no-one did it. Instead, someone data-mined the code and got it that way. Apparently, attempting to figure out the puzzle alone would have taken years. Sod that.
