I had ivy growing up my side fence and I just severed all the stems near ground level and pulled up as many roots as I could find. The odd stem pops up but I am in the garden weeding regularly so just pull it up as it appears. Mimi's link is good

Japanese knotweed kills ivy, so you should plant loads of that around the base of the ivy.
Hack the fucker up as much as you can. When you get as much up from the root as possible, get a hatchet on what's left and pour this on it:
